Does ICBC actually pay for physiotherapy?
Yes — and it’s more straightforward than most people expect.
Since BC moved to the Enhanced Care model in May 2021, your right to recovery care after a crash no longer depends on proving who was at fault. If you were injured in a motor vehicle accident in British Columbia, ICBC provides a block of pre-approved treatment — physiotherapy, and depending on your injuries, kinesiology, massage therapy, and other care — that you can access right away.
You don’t need to win an argument, hire anyone, or wait for an adjuster to sign off before your first appointment. Pre-approved means pre-approved: you book and start.
ICBC sets the specific number of pre-approved visits and the time window they sit inside, and it reviews those figures periodically. For the allowance that applies to your claim, check icbc.com or ask us when you book — we verify it as part of setting up your file.
Do I need a referral or a lawyer to use my ICBC physiotherapy coverage?
No to both.
- No doctor’s referral. Physiotherapists in BC are primary-contact practitioners — you can book directly. For an ICBC claim, you do not need a GP to refer you before starting physio. (More in our guide on whether you need a doctor’s referral to see a physiotherapist in BC.)
- No lawyer. The pre-approved treatment is a benefit you’re entitled to under Enhanced Care, independent of any legal process. You’re welcome to seek legal advice for other parts of a claim, but you do not need it to start your recovery care.
What you do need is a claim number. Report the accident to ICBC (online or by phone), and they’ll open a claim and give you a number. Bring that number to your first appointment and we take it from there.
How does direct billing to ICBC work at Envision?

- Report the crash to ICBC and get your claim number.
- Book your assessment at South Granville or False Creek — bring your claim number and the date of the accident.

- You focus on recovery, not paperwork. Your Envision physiotherapist will send in your treatment plan. If you require extended coverage after your first treatment block runs out, your physiotherapist will submit a report to ICBC requesting whatever additional coverage is needed.
What injuries from a car accident does physiotherapy treat?
The crashes we see at our two Vancouver clinics rarely come down to one tidy injury. But there are some more common conditions:
- Whiplash and neck pain — the classic rear-end-collision injury. Often shows up a day or two later, not at the scene.
- Concussion — you do not need to have hit your head; a whiplash mechanism alone can cause one. Concussion plus cervical (neck) physiotherapy is one of the highest-yield interventions in the first 90 days after a crash. - Back pain — neck, mid back or lower back strain from the impact and the bracing your body does in the half-second before it.
- Shoulder, knee, and soft-tissue injuries — from seatbelt loading, airbag contact, or bracing against the wheel.
The reason early assessment matters so much here: many of these injuries are most treatable in the first few weeks, and some — concussion especially — recover faster when active rehabilitation starts early rather than after weeks of waiting to “see if it settles.”
The first 48 hours post injury can be quite acute, and inflammation can be high. Often, at this stage, gentle mobility exercise but rest from more strenuous aspects of life is helpful. After 48 hours, early treatment can be helpful — both to deal with the injuries, but also to provide guidance into the best course of action to promote recovery.
How soon after the accident should I start physiotherapy?
As soon as you reasonably can — ideally within the first week or two.
Two things are working against waiting. First, the clinical one: injuries like whiplash and concussion respond best to early, active treatment; passive rest-and-wait tends to lengthen recovery. Second, the administrative one: ICBC’s pre-approved treatment sits inside a time window, so a block of visits you don’t use early can be harder to make full use of later.

I was in a crash weeks ago and only now have symptoms — is it too late?
Not necessarily. Symptoms like whiplash and concussion commonly surface days after a crash. Report the accident to ICBC and book an assessment. The sooner you start the better, but late is still worth doing.
